You will want to add your unit's IU marketing lock-up to your Qualtrics account and forms. The marketing lock-up includes the trident and your unit's name. Please contact our office at collcomm@iu.edu if you need to locate your marketing lock-up.
On the survey (form) page, select the paintbrush, which allows you to adjust the Look and Feel.
On the Theme menu, under Dynamic Themes, choose IU (add your own logo) and then Logo under the Theme menu.
On the Logo Image, select Change.
You should now see a window labeled Insert Graphic. Select the green Upload a New Graphic Button.
Choose your marketing lock-up file to upload and click the green Save button.
Click the blue Apply button in the bottom right hand corner of the screen.
