Form solutions
For most groups, the College is recommending Qualtrics to replace MachForm as a form solution because it is easier to use than Fireform. However, Fireform is an option if you have complex form needs.
If you wish to go the Qualtrics route, the first step is to email CITRIS@iu.edu to request an IU group account that you will use for your unit's Qualtrics account. College IT will assist you in setting this up. Once you have your group account, please contact our office.
Qualtrics form templates
Our office has created Qualtrics templates for some commonly used forms. We hope these templates will make the MachForm to Qualtrics migration process easier and more efficient for our departments and programs. Additionally, using similar language on common forms will help to provide a better user experience for our students.
Qualitrics has an online support portal, and we will continue to add resources about web forms at IU as they become available.
You may also build your own custom forms in your Qualtrics account. Please let us know if you have a suggestion for another template form.
